Gum disease could cause Alzheimer’s disease, new study claims

Alzheimers disease could be caused by a gum infection, according to a new study.

The study, published this week in the peer-reviewed journal Science Advances, suggests the bacteriaPorphyromonas gingivalis that destroys gum tissue in the mouth is linked to dementia and Alzheimer's.

Researchers observed the bacteria in the brains of people with Alzheimer's disease. They also conducted tests on mice that showed the gum infection led to an increased production ofamyloid beta, a part of theamyloid plaques associated withAlzheimer's disease.

"Despite significant funding and the best efforts of academic, industry, and advocacy communities, clinical progress against Alzheimer's has been frustratingly slow,"Casey Lynch,author on thepaper and CEO of pharmaceutical company Cortexyme, said in a statement. "TheScience Advancespublication sheds light on an unexpected driver of Alzheimer's pathology."

Cortexyme, which funded the research, is designing a series of therapies to treat the gum infection that plan to go to Phase 2 and 3 clinical trials.

While there have been previous studies linkingperiodontal disease and Alzheimers, researchers who aren't affiliated with the paper say there isn't clear enough evidence linking the two.

"In research weve supported to uncover the key risk factors for Alzheimers disease, gum disease hasnt emerged as a major cause for concern,"James Pickett, the head of research at Alzheimers Society who was not involved in the paper, said in a statement.

No cure currently exists for Alzheimer's disease, the most common type of dementia. The disease that begins with memory loss affects as many as 5million Americans, according to the Centers for Disease Control and Prevention.

Study links gum disease-causing bacteria to Alzheimer’s

Researchers say they've found more evidence linking bacteria found in a common type of gum disease to dementia. A new study, published in the journal Science Advances, found a key pathogen associated with chronic periodontal disease in the brains of people with Alzheimer's disease.

The study authors say these results, plus additional testing in mice, provide "solid evidence" of a link between the two diseases and may offer a potential new way to treat Alzheimer's. The devastating illness affects 47 million people worldwide, and there is no cure.

However, Alzheimer's experts not involved in the research caution that it is too early to tell how strong this association is, or whether it could lead to effective treatments.

Scientists from Cortexyme, Inc., a privately held, clinical-stage pharmaceutical company, analyzed the brain tissues of patients with Alzheimer's disease and found evidence of Porphyromonas gingivalis, the bacteria associated with gum disease.

Further tests in mice found that this bacteria could travel from the mouth to the brain and increased production of amyloid beta, a protein strongly associated with Alzheimer's.

In addition to the Porphyromonas gingivalis, the researchers found toxic enzymes produced by the bacteria called gingipains in the neurons of patients with Alzheimer's. Higher levels gingipain were associated with tau and ubiquitin, two other proteins involved in the development of Alzheimer's.

The team also tested drugs in mice aimed at clearing the harmful bacteria and blocking its toxic enzymes. In these lab experiments, scientists were able to reduce the number of Porphyromonas gingivalis bacteria in the infected brains, block the production of toxic proteins, and halt degeneration in the brain.

The researchers hope this will provide the basis for developing a new therapy that could one day treat humans in a similar way.

"The findings of this study offer evidence thatP. gingivalisand gingipains in the brain play a central role in the pathogenesis [development] of AD [Alzheimer's disease], providing a new conceptual framework for disease treatment," the study authors write.

This is not the first study to show a relationship between gum disease and Alzheimer's.

A 2017 study out of Taiwan found that people with chronic gum disease lasting 10 years had a 70 percent increased risk for developing Alzheimer's. Another small study published in 2016 in thejournal PLOS ONE found gum disease was associated with a six-fold increase in the rate of cognitive decline in people with mild to moderate dementia.

Other research has looked at whether various bacterial, viral, or fungal infections may play a role in Alzheimer's, but there is currently not enough evidence to say.

"The idea that bacteria and viruses may play a part in brain disease like Alzheimer's is not necessarily new," Rebecca Edelmayer, Ph.D., director of scientific engagement for the Alzheimer's Association, told CBS News. "But what this paper suggests is really an association and not causation and that should be very clearly emphasized when we're talking about studies like this. More research is needed to really identify a causative role for microbes."

In other words, it's unknown whether gum disease increases the risk of Alzheimer's or if people with dementia have an increased risk of gum disease because of poor oral care.

Edelmayer also cautions not to put too much weight on drugs tested in mice. "It really will be important to see how this plays out in human randomized controlled trials, which is the gold standard for understanding whether a therapeutic targeting something like the P. gingivalismechanism would actually be effective," she said.

While taking care of your teeth and gums is an important part of healthy aging, Edelmayer says it's too early to say if those steps could help prevent Alzheimer's disease.

Of course, there are many factors that go into someone's individual risk of developing dementia. The Alzheimer's Association has a campaign called"10 Ways to Love Your Brain" to help people lower their risk.

"We think a lot about things like diet, exercise, a good management of cardiovascular health, getting good sleep. All of these things could potentially play a role as a lifestyle intervention for decreasing your risk of developing cognitive decline," Edelmayer said.

She notes that good oral health could one day fall into one of these categories if there's more evidence to show that gum disease actually increases the risk for developing dementia.

Still, Edelmayer says with so much still unknown about the disease, studies like this are important for gaining a better understanding of Alzheimer's.

"I think the research really reinforces the complexity of Alzheimer's disease," she said. "It highlights the importance of sharing information and experiments and data like this very freely and widely across the research community so this robust discussion about this kind of science takes place."

A useful line of analysis is to consider the effect of scale changes for creatures which aresimilar in shape and only differ in scale. As the scale of an animal increases the body weight and volume increase with the cubeof scale. The volume of blood flow required to feed that bulk also increases with the cube of scale. The cross sectional area of the arteries and the veins required to carry that blood flow only increases with the square of scale. There are other area-volume relationships which impose limitations on creatures. Some of those area-volume constraints, including the above one, are:

Thus to compensate for the body needs which increase with the cube of scale but the areas increase with only the square of scale the average blood flow velocity must increase linearly with scale. Blood flow velocity is driven by pressure differences. The pressuredifference must be great enough to carrying the blood flow to the top of the creature and great enough to overcome the resistance in the arteries and veins to the flow. The pressure required to pump blood from the heart to the top of the creature is proportional to scale.The pressure difference required to overcome the resistance to flow through the arteries intothe capillaries and back again through the veins is more difficult to characterize in terms of scale.The greater cross sectional area reduces the resistance but the long length increases resistance. The net result of these two scale influences seems to be that the pressure difference required to drive the blood through the bulk of the creature is inversely proportional to scale. The pressure difference imposed would be the maximum of the two required pressure differences.

Open 24 hours a day and 365 days a years, Anytime Fitness is a different kind of fitness center. Anytime Fitness utilizes a franchising approach that offers entrepreneurs a convenient and affordable recurring-revenue business model. The first Anytime Fitness gym opened in May 2002 in Cambridge, Minn. It has opened an average of 300 new gyms each year for the past eight years. I joined Anytime Fitness in 2010, says Michael Zwiefel, director of construction and design. When I started, the company had just launched a new dcor package to create a unified brand image for their clubs. My role was to lead implementation of the program for new clubs as well as our existing facilities.

Anytime Fitness works with its franchisees to guide them through the design of their club and educate them on the overall development process. Its franchisees come from all walks of life and have varying experience within the industry.

The big challenge for us is to help them through that process, Zwiefel says. Our franchisees manage the development and construction of their gyms. The process can be complicated for a first-timer. We provide tools, support and guidance to ensure they are successful.

What can be both a blessing and a curse for the company is the fact that the model offers a degree of flexibility. We are not a cookie-cutter concept with a rigid operating platform, Zwiefel says. While we have core requirements and standards we still allow for flexibility in some of the amenities and design. This flexibility can be a challenge to manage from a design and brand aspects, but it is also one reason why we are so successful. We look at each location individually, ensuring the basic requirements are met from a functional and branding perspective. The franchisee is involved throughout the process and are able to influence many components of the design.

Anytime Fitness franchises typically opens in existing real estate, which results in a wide variety of spatial configurations. The lack of ground-up construction makes it nearly impossible to maintain a true prototype.

Anytime Fitness keeps its finger on the pulse of the fitness industry as well as trends in construction, design and materials. The company looks for the right vendor partners that can provide the true value.

We are always concerned with cost, but it is only one aspect of material and vendor selection Zwiefel says. Value is key. First and foremost, we have to pick the right products for the intended use and ensure they perform. Price, ease of installation, durability, maintenance, warranty and cost of owner all go into material selection. Ensuring we are partnering with the right vendor is also important. Factors such as points of distribution, shipping, ease of ordering and customer service are critical, especially with a franchised concept that covers a larger geographic area.

Since he came on board in 2010, Anytime Fitness has supported Zwiefels department by adding a number of team members to ensure the company can support its operations. In addition to making sure the company has the right vendors in place and continues working to stay on top of trends, Anytime Fitness also has invested in technology to help franchisees operate the brand and connect with members.

About 80 percent of all Anytime Fitness gyms are located in the United States. But already international locations have opened in Canada, Mexico, Australia, New Zealand, England, Scotland, Ireland, Wales, the Netherlands, Poland, Qatar, Grand Cayman, India, Chile, Spain, Japan, Singapore, Hong Kong, Malaysia and the Philippines.

Within five years from now, the company anticipates that its U.S. portfolio will be closer to 50 percent of its footprint with approximately 4,000 clubs in the United States and another 4,000 around the globe.

We want to maintain a vibrant and successful global model, Zwiefel says. This is a fast-paced and ever-changing industry. Toolbox Squat & Raise (10-20 reps)Hold the toolbox out in front of you and squat from the knees ensuring you keep your arms out in front of you, back straight and feet flat. Stand back up and raise the toolbox above your head before repeating.

Pipe Clean & Press (10-20 reps)Start with the pipe on the floor and bend down keeping your back straight. Lift the pipe to chest height and get comfortable with the weight before pressing it overhead with straight arms.

Bumper Dips (10-20 reps)With the rear doors open to place your hands on the bumper and stretch your legs out in front of you. Lower yourself down until your elbows reach 90 degrees or you touch the floor.

Seated Wrench Curls (10-20 reps)Open the side door and sit on the edge. Take a wrench and lift from your waist to your shoulder keeping your body still. Repeat for a wrench in each hand.

Panel Push-Ups (10-20 reps)Place your hands on the bumper facing the van in a regular press-up position with your legs straight out behind you. Lower yourself toward the bumper before lifting yourself back up.

Tancred (1995)[1] believes that: "One of the misconceptions in the sports world is that a sports person gets in shape by just playing or taking part in his/her chosen sport. If a stationary level of performance, consistent ability in executing a few limited skills is your goal, then engaging only in your sport will keep you there. However, if you want the utmost efficiency, consistent improvement, and balanced abilities sportsmen and women must participate in year-round conditioning programs. The bottom line in sports conditioning and fitness training is stress, not mental stress, but adaptive body stress. Sportsmen and women must put their bodies under a certain amount of stress (overload) to increase physical capabilities."

The World Health Organization (WHO) defined health in its constitution of 1948 as "a state of complete physical, mental, and social well-being and not merely the absence of disease or infirmity".

Fitness is the ability to meet the demands of a physical task.

Basic fitness can be classified into four main components: strength, speed, stamina and flexibility. However, exercise scientists have identified nine components that comprise the definition of fitness (Tancred 1995)[1]:

Dementia Australia | Alzheimer’s disease

Alzheimers disease is the most common form of dementia, affecting up to 70% of all people with dementia. It was first recorded in 1907 by Dr Alois Alzheimer. Dr Alzheimer reported the case of Auguste Deter, a middle-aged woman with dementia and specific changes in her brain. For the next 60 years Alzheimers disease was considered a rare condition that affected people under the age of 65. It was not until the 1970s that Dr Robert Katzman declared (rather boldly at the time) that "senile dementia" and Alzheimers disease were the same condition and that neither were a normal part of aging.

Alzheimers disease can be eithersporadicorfamilial.

SporadicAlzheimer's disease can affect adults at any age, but usually occurs after age 65 and is the most common form of Alzheimer's disease.

FamilialAlzheimers disease is a very rare genetic condition, caused by a mutation in one of several genes. The presence of mutated genes means that the person will eventually develop Alzheimer's disease, usually in their 40's or 50's.

The whole brain is fed and provided with oxygen by a complex network of arteries, veins and capillaries. This vascular network is strictly controlled and segregates the brain from the rest of the blood stream. The blood brain barrier protects the brain from infection, but consequently if the brain does become infected it is difficult to treat, as many antibiotics are too large in their molecular structure to cross the barrier. This is also a major problem when finding agents to treat Alzheimers disease, as they must pass this barrier to target the brain.

Behind the ears and temples are the temporal lobes of the brain. These regions process speech and working memory, and also higher emotions such as empathy, morality and regret. Beneath the forebrain are the more primitive brain regions such as the limbic system. The limbic system is a structure that is common to all mammals and processes our desires and many emotions. Also in the limbic system is the hippocampus a region that is vital for forming new memories.

The cerebellum is at the back of the brain, which stores our muscle memory so we can do things without thinking - such as riding a bike. The midbrain and brain stem are the most primitive regions of the brain. They control bodily functions such as heart rate and digestion and act as an interface between the spinal cord and the rest of the brain.

All these complex tasks are mediated by the connections between the brain cells (neurons) called synapses. In the adult human brain there are around 100 billion brain cells, each connected to its neighbours by 5-10,000 synapses.

Our brains form a million new connections a million new synapses every second we are alive. The pattern and strength of the connections is constantly changing and no two brains are alike.

It is in these changing connections that memories are stored, habits learned and personalities shaped, by reinforcing certain patterns of brain activity, and losing others.

Brain cells communicate though synapses in a variety of ways. Signals pass move across the synapse in the form of chemicals that are known as neurotransmitters. Neurotransmitters a passed from one brain cell, across the synapse (connection) and to the receiving brain cell, which collects the neurotransmitter with a receptor. The receiving cell can then send out another burst of neurotransmitters to other brain cells to pass the message on.

Going back to the 1900s, Dr Alzheimer examined the brain of his patient, Mrs Deter, upon her death. He found shrinking of the outer layer of the brain or cortex the region of the brain involved in memory, language and judgment. We know that the so called shrinking of the brain is caused by the death of the brain cells.

Dr Alzheimer also found two types of deposits in Deters brain. One kind was found outside the brain cells, which are known plaques and the other type of deposit was found inside brain cells known as neurofibrillary tangles. These plaques impair synapses so signals cannot pass between brain cells. Tangles kill brain cells by preventing the normal transport of food and energy around the brain cell.

As brain cells die the brain shrinks, which can be detected using imaging such as magnetic resonance imaging (MRI).

The outer part of the brain is usually the area affected first by the disease. Short-term memory loss is therefore one of the first symptoms of Alzheimers disease. But as the disease progresses to deeper parts of the brain, long-term memory is also lost. The disease also affects many of the brain's other functions and consequently, many other aspects of behaviour are disturbed.

Apart from the few individuals with Familial Alzheimer's disease, it is not known why one individual gets Alzheimer's disease late in life and another does not. Scientists are investigating what triggers the formation of plaques and tangles and about other chemical changes that damage brain cells in Alzheimer's disease.

A variety of suspected causes are being investigated including factors in the environment, biochemical disturbances and immune processes. The cause may vary from person to person and may be due to one factor or a number of factors.

In the early stages the symptoms of Alzheimer's disease can be very subtle. However, it often begins with lapses in memory and difficulty in finding the right words for everyday objects.

Symptoms vary and the disease progresses at a different pace according to the individual and the areas of the brain affected. A person's abilities may fluctuate from day to day, or even within the one day, becoming worse in times of stress, fatigue or ill-health.

The rate of progression of the disease varies from person to person.

However, the disease does lead eventually to complete dependence and finally death, usually from another illness such as pneumonia. A person may live from three to twenty years with Alzheimer's disease, with the average being seven to ten years.

There is currently no single test to identify Alzheimer's disease. The diagnosis is made only after careful clinical consultation.

The clinical diagnosis might include:

These tests will help to eliminate other conditions with similar symptoms such as nutritional deficiencies or depression. After eliminating other causes, a clinical diagnosis of Alzheimer's disease can be made with about 80% to 90% accuracy if the symptoms and signs are appropriate. The diagnosis can only be confirmed after death by examination of the brain tissue.

It is important to have an early and accurate diagnosis to determine whether a treatable condition other than Alzheimer's disease, is causing the symptoms. If Alzheimer's disease is diagnosed, medical treatment and other assistance can be discussed.

At present there is no cure for Alzheimer's disease. However, one group of drugs called cholinergeric drugs appears to be providing some temporary improvement in cognitive functioning for some people with mild to moderate Alzheimer's disease.

Drugs can also be prescribed for secondary symptoms such as restlessness or depression or to help the person with dementia sleep better.

Community support is available for the person with Alzheimer's disease, their families and carers. This support can make a positive difference to managing dementia.
