Beware the Trojan Horse of Integrative Medicine | Office for Science and Society – McGill Newsroom

The story of the Trojan horse is well known: the Greeks allegedly delivered to the city of Troy a massive wooden horse, which the Trojans mistook for a gift and pulled inside their city. At night, this hollow horse released a band of Greek men who had been hiding inside of it, and they opened the city gates so that their army could strike the final blow in the Trojan War.

The concept of integrative medicine has gained in popularity since it was coined by Dr. Andrew Weil in 1994 and it is important to recognize it as the Trojan horse that it is. Although the metaphor usually implies deception, many fans of integrative medicine promote this gift horse without misleading intentions, but the damage may well be the same.

The claim at the heart of integrative medicine is that conventional medicine is not enough and that so-called complementary and alternative medicine (CAM) is also insufficient, but that by integrating the two, patients get the best of both worlds. Medicine is accused of being hyper-focused on disease and on the use of pharmaceuticals, failing patients with chronic illnesses. CAM is positioned as the answer to this, the yang to conventional medicines yin to yield a complete, holistic perspective.

On its surface, the CAM half of integrative medicine looks wonderful. We are usually told it involves nutrition and exercise. Similarly, proponents of integrative medicine claim its added value is in holism, meaning focusing on the whole person. Strangely though, as has been argued by many people, conventional medicine at its best is focused on the whole person. But integrative medicines seductive, superficial messaging does not end there.

This Trojan horse has been making major in-roads inside of academic hospitals, and it moves on four wheels: the appeals to nature, antiquity, authority, and popularity. Hospital directors and patients alike are told that integrative medicine prioritizes natural treatments... without mentioning that synthesized products are not necessarily harmful and natural ones, not necessarily harmless (or useful). They are told that many of these interventions, like acupuncture, have been used for a long time... just like bloodletting was in ye olden days. They are told that many serious university hospitals, like Johns Hopkins, Duke, and Yale, are already offering integrative medicine... but keeping up with the Joneses is no substitute for a critical appraisal of the body of evidence. And they are told that many, many people are clamouring for these therapies. Market forces being what they are, the Trojan horse rolls into town and we may wonder what pours out of it.

Inside the Trojan horse of integrative medicine, painted in the colours of nutrition and exercise, we find unproven and disproven remedies like homeopathy, the 200-year-old philosophy that claims that the more a substance is diluted, the stronger it becomes. We find Reiki and other energy healing interventions, which pretend that hands-off massages of an undiscovered force field around the body can provide healing. We find poorly regulated herbal remedies, problematic therapies like acupuncture, and things like reflexology, where a foot massage can somehow help your stomach heal itself. The horse also contains more benign interventions, like art therapy and massages, but the majority of CAMs contribution puzzles the mind. These often pre-scientific folkloric therapies often lack plausible mechanisms. Given our extensive knowledge of biology, it makes no sense for the entire human body to be represented on the sole of our feet (see reflexology). Given our extensive knowledge of chemistry, it makes no sense for vast dilutions that leave behind no trace of the ingredient to work (see homeopathy). Yet proponents often throw their hands up when confronted with this and simply claim that it works, how ever it may work.

Universities can easily fall under the spell of integrative medicine because of what I would call the two towers. Imagine two towers that visually represent the evidence we have for conventional medicine and for complementary and alternative medicine, things like homeopathy and Reiki. The conventional medicine tower has an old foundation and is continually being extended and repaired with better materials. By comparison, the CAM towers foundation is made of cheap, imitation material, and while theres a shell that gives it its full height, the bricks have not been laid yet. But the structure is draped in a banner that illustrates what the tower will look like when finished. From a distance, both towers appear similar. Same height, same look. But when you get closer to the CAM tower, you notice how grossly incomplete it is. Dont worry, you are told, what we have so far is very promising and we will keep building it. This slogan never goes away. The CAM tower will always be sold as promising, year after year, convincing many people to invest in it.

Studies of ear acupuncture, craniosacral therapy, homeopathy and many other complementary interventions are usually small, poorly done, and encouraging. When rigorous trials are completed, they fail to demonstrate efficacy, which leads CAM proponents to return to smaller studies and extract hopeful results from those. This tower of promising results can then be presented to academic health centres by philanthropists who believe acupuncture or homeopathy cured them, and their generous donations can lead to the creation of integrative medical centres within these hospitals. Impressive consortia are created to advocate for integrative medicine and to put pressure on medical school curricula to pull the Trojan horse in.

There are clear harms to this. Obviously, if I were to argue that because astronomy is insufficient, it needs to hold hands with astrology in university faculties, it would be easier to see the potential for intellectual harm. Similarly, I have seen Canadians elevating Indigenous ancestral knowledge to the same level as science and asking for its integration into medicine, without testing these claims with the most rigorous tools we have, and I find this equally troublesome

Given how many CAM disciples worship at the altar of Mother Nature and see toxins everywhere, a vote for integrative medicine often risks bolstering unnecessary vaccine hesitancy.

If you are caught looking this Trojan horse in the mouth, the most likely retort you will hear is that conventional medicine has problems. Yes, it does. It is true that medicine does not offer great solutions to many chronic conditions, chief among them chronic pain. Some of its solutions, like opioids, have also caused significant harm because of misplaced economic interests. But the medicine tower gets fixed. Moldy parts are extruded and replaced. Meanwhile, the CAM tower remains deeply flawed and mostly illusory. The bricks are coming, we are told, and soon we will have the proof we need.

If I may bring one more metaphor to this crowded landscape, it would be Dr. Ben Goldacres pearl of wisdom. Problems in aircraft design, he says, do not mean that magic carpets can actually fly. As the Trojan horse of integrative medicine knocks on the doors of our institutions, we must remember that what we need is not a hollow prize with a corrupting cargo; we need to keep fixing the real tower.

A couple of weeks ago I started a series of articles focused on our food production system in the United States and common questions that arise regarding how our food makes it to the grocery shelf. Previous articles have focused on organic production and the use of GMOs in the United States.

This all came about when I was visiting with someone during the market steer show during our county fair. The specific question that was asked was whether grass-fed beef was better for a person than beef that comes from cattle that is fed the more traditional grain finishing diet. So, for todays article this is the topic that I will focus on.

To give a little bit of background, grass-fed beef is a relatively new production system where cattle are fed a strictly forage-based diet. This is different than what is considered the traditional production system where beef animals are finished in a feedlot setting where they are fed grain-based diet.

As with any type of production method for anything, no system is perfect, and there are positive attributes and drawbacks to each. The reason that the traditional method (grain finished) is the commonly used method of finishing animals is because it is largely the most efficient method of doing so. Using the grain-fed system, these animals can be grown to an acceptable finishing weight in a shorter span of time and the meat from these animals will typically contain more intermuscular fat. Intermuscular fat or marbling leads to a better eating experience as meat with more marbling is generally more tender when cooked.

Part of the reason that grass-fed beef is not as popular as grain-fed is because the US consumer is more accustomed to eating beef from animals finished on grain. As I mentioned above this meat is generally more tender and provides a taste and tenderness profile that people are used to.

I hesitate to make generalizations but when comparing grain-fed to grass-fed; grass-fed beef will produce smaller steaks that have less marbling but also less external fat. In addition, because of the makeup of the forage diet the fat on and in a grass-fed animal will tend to take on a yellowish-green hue. It is still fat and does not affect the eating experience, but it tends to look different. Grass finished beef also traditionally has a stronger beef taste.

A feather in the cap for the grass-based system is that beef raised using this system generally contains higher levels of Omega-3 fatty acids, higher levels of vitamins and minerals, and lower calories because of the lower fat content. Studies have proven these factors to be true, but, for the most part, these levels are either not significantly different or, as is the case with the vitamins and minerals, traditionally raised beef already provides what our bodies are able to use, so the increase does not really provide much.

As with most production systems, preference comes down to personal choice. If you are considering switching from grain-fed beef to grass-fed, or vice versa, I think the important thing to remember is that you may not be getting the exact same eating experience that you used to get, and it may cost more. In the end, however, it is still beef.

Chefs join SIUE nutrition students to share their expertise

EDWARDSVILLE In a time of virtual everything, students can now add online cooking classes to their calendar.

Southern Illinois University Edwardsville nutrition students are hosting a series of cooking classes via Zoom throughout November with extraordinary guest chefs.

Participants will create and share a full course meal with an appetizer, entre and side dish. Visiting chefs will offer real-time instruction ranging from basic cooking skills to gourmet meal preparation.

Our students continue to demonstrate their ingenuity as we engage in online teaching and learning amid the pandemic, said Kathy Mora, PhD, RD, assistant professor in the Department of Applied Health. We are excited to offer fun, engaging classes that feature the culinary expertise of some wonderful local chefs. We appreciate their willingness to partner with us for this unique, hands-on experience.

The first class is scheduled for Monday, Nov. 2 beginning at 5 p.m. Chef David Kirkland of Turn Restaurant in St. Louis will be the guest chef. The theme is Harvest Fiesta, and will feature a menu of Chicken Posole, Sikil Pak (toasted pumpkin seed dip), guacamole and lime cranberry margaritas or mocktails.

Interested participants can register at http://www.enrole.com. Registrants will receive an ingredient list and a link to join the Zoom meeting via email prior to each course.

The next class is slated for Monday, Nov. 9 with Chef Niall Campbell from Firefly Grill in Effingham, with the theme Creole Birthday Bash. On Monday, Nov. 16, the theme will be Allergy Friendsgiving, with representatives from UKraft Food Truck out of St. Louis.

Eating well on the golf course is one of the most underrated ways to improve your score.

It was about this time last year when I decided to lose some weight. There was no real reality check moment for me; rather, it was a slow burn. More and more people seemingly all at once started referring to me as big guy hey, big guy! which caught me off guard. And then there were the times when I felt generally uncomfortable in my own body, which were growing more and more frequent. It was most notable when I was swinging a golf club. It was like I wasnt or maybe couldnt move as nimbly as I needed to.

So I shook things up. On the left I clocked-in at just north of 211; in the picture of me from two weeks ago on the right, Im maintaining my goal weight of about 163.

The process of losing weight is actually very simple. You eat less of some things and more of others. You can read about all the stuff I learned right here, but ultimately its about the choices you make and your ability to stay disciplined. My favorite Bojack Horseman quote of all time pretty much sums it up:

Every day it gets a little easier, but you gotta do it every day, thats the hard part.

Something that was indispensably important to me throughout it all was WW, formerly Weight Watchers. I use the app to track my food and genuinely love it, because the system isnt about counting calories or telling you what you cant eat, but rather helping you manage what you do.

And so, to glean some advice along those lines for all us day-in, day-out golfers out there, I called up WWs Head of nutrition & wellness Jaclyn London, MS, RD, CDN, who shared a host of helpful tips for golfers about the kinds of foods they should eat on the course, whether they want to lose weight or not.

Ive often wondered whether drinking coffee is conducive to a sport like golf, where the rounds are long and the movements often require a calm, precise touch. But far from not being detrimental to your game, London says coffee can be a good for golfers, consumed at the right time and in the right dosage.

A pre-round cup of coffee is a great way to up your energy level. It can contribute to your hydration goals and help you stay alert during your round, London says, who points to the Dietary Guidelines for Americans of up to 400 mg of caffeine daily, and recommends finishing your coffee up to an hour before your round.

Changing your coffee order to a 16 ounce latte, with low fat and skim milk, can also provide up to 16 grams of protein which will help you sustain that energy.

You donthave to eat breakfast, London says, but she recommends it. Not just because its an easy way to up your energy levels, but because its a healthy habit: Skipping breakfast means your hunger will come back to haunt you later, and as the desperation to get something in your stomach mounts, youre more likely to gorge on anything you can find.

The key, London says, is to use food as fuel to maintain a nice, steady energy level throughout your entire round.

Establishing a pattern right off the bat of not skipping meals in the a.m. sets you up to make better choices all day even if youre not ravenous when you wake up, London says. Eating within 30 min. to one hour after waking up can help you get the day started and can provide you with some much-needed fuel in the tank.

Complex carbohydrates arent bad, despite what you may have heard, but its important to understand what they do best. In short: Carbs provide quick, intense jolts of energy. That can be extremely helpful for golfers, London says, as long as they stick to a 2-to-1 protein-to-carbs ratio.

The glucose from the carbs gets into your bloodstream really fast, London said. You need the combination of carbs and protein, making sure that you maintain energy and feel satisfied throughout your round.

Snacking throughout your round is another healthy habit recreational golfers should take on board. And when they do, its important they look for higher-protein snacks that will provide some sustenance and steady energy.

Dried cheese snacks like dried parmesan, a piece of fruit or a pack of dried unsweetened fruit; a pack of nuts or nut butters, are all great options, London says. Youre looking for mini-meals with protein, fiber, and healthy fats.

Drinking enough water on the golf course, especially during the hot days, is perhaps the single most important thing you can do. The benefits are almost innumerable: It improves your focus, maintains your energy levels, and aids in digestion, among many other things. And when youre playing golf, you tend to lose water more quickly, so you need to replenish consistently.

Hydration helps with just about everything, London says. It changes depending on your climate, but I use eight-to-10 glasses per day as a starting point for an active person.

Therell be days when you fall off the wagon. Youll stick around after your round, share a few beers and share a plate of something. Its inevitable, and when it does happen, its important to dwell on it. Move on and get back to your consistent routine, based around what London calls the three Ts

I often say progress is better than perfection, London says. Practice making good choices so the the next time youre coming into those type of situations, youre not ravenous, but coming in from a hydrated, satisfied place where you dont have as much desire to eat all the cheese fries.

Yuvraj Singh invests in nutrition-based healthtech startup Wellversed – Business Insider India

During my battle with cancer, I realised the importance of maintaining a good nutrition regime for our overall health and wellness. Wellversed is redefining the way nutrition and food products are crafted. It is not just another nutrition brand to me, its a technological innovation that paves the way for a fundamental change in the entire nutrition ecosystem, said Yuvraj Singh, Founder, YWC Ventures in a statement.

Singh founded YWC Ventures in 2015 and since then has invested in several startups like Healthians, EazyDiner, and the likes of salon chain Naturals.

The company said that almost half of its revenue (40%) is through direct to consumer selling through its website. Its products are also available on e-commerce sites like Amazon, BigBasket and others.

With the latest investment, the company now plans to expand to newer markets, invest in technology and research and development.

Required Revisions to Nutrition and Supplement Facts Labels are to be Fully Implemented by January 1, 2021 – JD Supra

The Food and Drug Administration issued new regulations requiring changes to the nutrition and supplements facts labels of conventional food and dietary supplements, respectively, on May 27, 2016.1 Eventually, implementation dates for these new regulations were set so that manufacturers with $10 million or more in annual sales were required to switch to the new label by January 1, 2020, and manufacturers with less than $10 million in annual food sales have until January 1, 2021, to comply.2 FDA also announced that it intended to exercise enforcement discretion to work cooperatively with manufacturers and will not focus on enforcement actions during 2020 for larger manufacturers and during 2021 for smaller manufacturers.3

Manufacturers and distributors qualifying for the first implementation date, January 1, 2020, should already have completed a review of all product labels, identified changes required to those labels in order to comply with the new regulatory requirements, and be actively ensuring that products labeled on or after the implementation date are compliant. Manufacturers and distributors required to satisfy the new label requirements by the second implementation date, January 1, 2021, at a minimum, should have initiated this same process. While we are all coming forward to realise the importance of health and wellness, Pinkvilla took a step forward and gave a platform to experts from various fields of the healthcare industry to talk about all the things that matter.

In the digital conversation held today, we had our minds blown as Dr Karishma Vijan, Dr Kiran Coelho, Dr Gittika Mitta Gupta, Ms Anali Mukerjee and Ms Shrima Rai spoke about their respective fields.

The conversation started off with Dr Kiran Coelho explaining all about PCOS and how a bad lifestyle and stress can result in a change of hormones in a womens body. Adding to it, she suggested that a proper diet backed with the right sleeping schedule, a healthy lifestyle and at least 30 minutes of exercising can reverse its effects.

Adding to it, she spoke about how 70 to 80% women go through PMS and ways to deal with it in a healthy manner.

Ms Anjali Mukerjee threw light on the benefits of a proper diet on PCOS and how low glycemic index diet can result in a healthy lifestyle. Vitamin supplements can further help in getting your lifestyle back on track.

Dr Geetika Gupta further talked about a 360-degree approach to dealing with any issues like PCOS at hand. You can never deal with acne by using topical products unless the insides are clean!

The conversation further delved into the areas of oral health, anti-ageing remedies, fertility and immunity boosting drips.

When I am stumped to remember a persons name, I try to kick it into my brain by going down the alphabet. Ann? Bonnie? Cathy? And so on. Sometimes it works

Speaking of the alphabet, here is the final round of good foods, starting where we left off last week:

Strawberries: One cup of this red fruit more than meets our vitamin C requirement for the day. Strawberries are also rich in substances that may lower our risk for developing Alzheimers disease, according to some studies. Keep strawberries as dry as possible, say the folks at Driscolls. Store them in their original container between 32-34 degrees F. Rinse them gently with cool water right before you are ready to eat them. And let them reach room temperature before serving to enhance their natural flavor.

Tea can sooth tired minds and adds fluid to tired bodies. Tea drinkers tend to have lower blood pressures and a reduced risk for strokes, according to some studies. Interesting, too, that green, black and oolong teas are made from the same plant, Camellia sinensis. They are just processed differently. Cup for cup, black tea has about half the caffeine of coffee while green and oolong varieties have less.

Udon. I turned to Marilyn Uwate, my dietitian friend with Japanese roots, on this one. Udon is a noodle made from wheat flour and widely used in Japan, she says. It is usually eaten as a soup. Look for whole grain versions for a better boost of nutrients.

Vegetables! Plants we eat for food are as varied as the nutrients they supply. Think of vegetables as natures vitamin, mineral and fiber supplements. For best health, let this group of foods grace your table often.

Walnuts. Besides being the nut with the highest amount of plant-based omega-3 fats (the good-for-you fat), walnuts are certified by the American Heart Association as a heart-healthy food. Before you eat the whole bag, remember that a serving is considered 12 to 14 walnuts halves or 1/4 cupabout a handful.

Xanthan gum might not be on your shopping list but is a common ingredient in salad dressings and sauces. According to the International Food Information Council xanthan gum helps stabilize opposing ingredients like oil and water. This food additive is also gluten-free so has become a useful way to mimic the properties of wheat flour in gluten-free breads and pastries.

Yogurt! A staple food in many cultures, yogurt is a great source of essential protein and calcium. And because it is fermented, it helps to keep healthful bacteria in our guts. Scientists now believe that good gut bacteria not only helps fight off infections but may also protect us from intestinal disorders and help with weight control.

Zucchini. Ta da! Dont make fun of this incredibly versatile summer squash. Its considered a non-starchy vegetable with only 25 calories and 5 grams of carbs per cup. I like it roasted or sauteed with a little olive oil and seasonings.
